Who’s Actually Handling Your Parent’s Care?
Family-owned, RN‑led team
Canyon Care Management is run by a family of Registered Nurses with decades of combined hospital experience, built on patient safety and advocacy.
Coordinators who speak the hospital’s language
Coordinating between doctors, specialists, and insurers is our daily clinical work. We use that same skill to manage your parent's appointments, portals, and paperwork so nothing falls through the cracks.
Deep clinical insight, not just paperwork
We've spent decades at the bedside working alongside physicians, therapists, and case managers. We understand the why behind every order, medication, and follow-up.
Frequently Asked Questions
-
Four common options, four gaps:
Non-clinical care managers and concierge services. There is no license or minimum qualification required to call yourself a care manager or patient advocate. Many have never worked in a hospital. Every Canyon Care nurse is a California licensed RN, with 40+ years of combined bedside experience. We can read the chart, question the medication list, and catch the red flag before it becomes an ER visit.
Hourly private advocates. At $150 to $400+ an hour, the meter is always running and families ration their own calls. Our pricing is flat.
Home care agencies. They recommend their own staffing because that is what they sell. We provide no home care. We pull around 15 quotes and hand you the best one.
Medicare sponsored care coordination. It sounds free, and there is a reason. Reimbursement is capped at roughly an hour of coordination per month, and the pay drops for time beyond that. So the program is built to ration your time, not solve your problem.
-
If you manage care more than you should, It slowly pulls on your attention and becomes a second job.
This lets you buy back peace of mind and the time to do more valuable things. That means fewer late‑night worries, fewer mistakes, and more time to be a son or daughter instead of the full‑time care coordinator.

This is built for professionals who are the “everything person” for an aging parent. Situations where you're juggling work, kids, and a growing list of medical fires. If you’re thinking “I can’t drop one more ball,” you’re the person this was designed for.
-
Neither. We are the layer between them. Your parent keeps their own doctors, and we make sure those doctors are talking to each other, that nothing gets lost between visits, and that someone with clinical training is reviewing the plan. We also do not staff home care, which is exactly why our recommendation is honest. When aides or home health are needed, we source around 15 quotes and hand you the best fit at the best value, with no stake in which one you pick.
-
That works. Most of what we do is done by phone, portal, and paperwork: coordinating providers, chasing records, fixing billing, and staying in contact with your parent and their care team. Distance does not change that. The one limit is in-person work. Outside our local area we cannot do home safety walkthroughs or attend appointments in person. If your parent is out of state, we will tell you upfront what we can and cannot cover.
-
No. We are private pay, and that is deliberate. Medicare's care coordination benefit pays for roughly an hour a month, which means your time is rationed by a billing code rather than by what your parent actually needs. Working directly for you means no caps, no prior authorization, and no third party deciding when we stop. You get flat monthly pricing instead of an hourly meter.
-
Book a call. We will talk through your parent's situation and tell you honestly whether we can help. If it is a fit, you sign a service agreement and a HIPAA authorization so we can speak with providers on your behalf. From there we run a deep intake with you and your parent, and you get a written 90-day plan covering what we are fixing first, second, and third.
