Most families learn how the medical system works during the worst week of their lives, and then forget it by the time the next person needs it. We think that's backwards. Our job is to make one family's hard-won knowledge into something the whole family holds.

We are nurses. We have spent our careers on the inside of hospitals, watching what happens after the discharge papers are signed and the family drives home with a folder they can't read.
The medicine is usually fine. What fails is everything around it: the referral nobody placed, the medication nobody reconciled, the denial nobody appealed, the specialist who never saw the other specialist's notes. Families end up doing skilled coordination work with no training, no time, and no one to ask.
Canyon Care was built to be that person. Not another agency that adds to the pile, but a single clinical team that takes the pile.

Every client is handled by a registered nurse licensed in California. We read the chart, understand the diagnosis, and ask the questions a non-clinical coordinator wouldn't know to ask.

One flat price covers the work. When a crisis demands more hours, we put in the hours and your bill stays the same. Nobody should hesitate to call us because of what the call costs.
No conflicts of interest
We don't own home care agencies or take referral fees from facilities. When we recommend a provider, it's because we compared the options and that one was best for you.
Plain language, always
If a family can't repeat the plan back in their own words, we haven't finished explaining it. Clarity is the deliverable.
